The Care Coordinator plays a crucial role in ensuring our care teams are deployed effectively, resources are optimized, and communication flows seamlessly between field teams, telehealth providers, and patients. Youโ€™ll manage day-to-day logistics, coordinate patient and clinician schedules, and be the hub that keeps operations running smoothly for acute-level care delivery.


๐Ÿ“‹ Key Responsibilities

  • ๐Ÿš‘ Coordinate deployment of field care teams across multiple markets based on patient needs and clinician availability.
  • ๐Ÿ“… Manage daily logistics: patient scheduling, resource planning, and operational troubleshooting.
  • ๐Ÿ’ฌ Serve as the communication hub between field teams, telehealth providers, and patients.
  • โš ๏ธ Monitor & respond to field safety alerts, escalating issues in real time.
  • ๐Ÿ’ป Leverage multiple software platforms to perform duties efficiently.
  • ๐Ÿ“ž Assist with overflow inbound communications from patients or internal teams.
  • ๐Ÿ“ˆ Support process improvements and quality initiatives for care delivery.
  • ๐Ÿ—ฃ Communicate clearly and effectively with patients and providers.

โœ… Requirements

  • 2+ years of experience in a healthcare setting (ambulatory, home health, pre-hospital, etc.)
  • ๐Ÿ“ Strong documentation and communication skills
  • ๐ŸŒ Remote-ready with ability to work independently and collaboratively
  • ๐Ÿšจ Proven ability to escalate care or operational issues appropriately
  • ๐Ÿง  Calm under pressure with a strong sense of urgency
