Join myLaurel as a Clinical Liaison in Brooklyn, NY. We are seeking a mission-driven Registered Nurse (RN), Social Worker (LMSW/LCSW), or Paramedic to act as the primary bridge between hospital leadership and our innovative acute care-at-home program. In this high-impact role, you will drive Acute Care Management by identifying eligible patients and fostering relationships with Interdisciplinary Teams.

Compensation & Perks

  • Salary: $100,000 – $105,000 base salary + performance bonus.

Core Responsibilities

  • Stakeholder Engagement: Cultivate strong professional relationships with hospital leadership, case managers, and discharge planners to integrate myLaurel into care pathways.

  • Referral Growth: Proactively identify and develop new referral sources within the Brooklyn hospital system to expand our service line.

  • Utilization Review: Conduct high-acuity Clinical Chart Reviews to screen patients for eligibility and facilitate the enrollment process.

  • Patient Advocacy: Educate patients and families on the benefits of acute care-at-home and provide seamless Discharge Planning support.

  • Operational Integration: Collaborate with hospital staff and the myLaurel clinical team to ensure accurate EHR Documentation and patient handoffs.

Qualifications & Requirements

  • Active Licensure: Must hold a current, unrestricted New York State license (RN, LMSW/LCSW, or Paramedic).

  • Clinical Background: 4+ years of clinical experience in an Inpatient or Emergency Room (ER) setting.

  • Technical Savvy: Proficiency in Electronic Health Records (EHR) and Google Workspace (Docs, Sheets, Slides).

  • Adaptability: Ability to work flexible 8-hour shifts (e.g., 9:00 am – 5:30 pm or 11:00 am – 7:30 pm) to meet business needs.

  • Connector Mindset: Excellent communication skills with a knack for business development and relationship management.
