The Role:

The Associate Manager, Care Navigation supports the day-to-day operational management of the Care Navigator team, ensuring timely, compassionate, and consistent support for patients and caregivers navigating dementia care. This role focuses on frontline team supervision, process adherence, and operational execution to maintain high standards of patient experience and team performance. The Associate Manager escalates issues to leadership and supports continuous improvement initiatives.

Essential Functions:

Front Line Supervision & Support

  • Oversee daily activities of Care Navigators, including scheduling, task completion, and quality standards.
  • Provide coaching and support to team members, reinforcing Rippl’s values and operational expectations.
  • Assist with onboarding and training of new Care Navigators.

Workflow Execution & Daily Operations

  • Monitor operational workflows and care coordination activities to ensure consistency and accuracy.
  • Track completion of patient outreach, education, and follow-up tasks.
  • Support quality assurance activities by monitoring adherence to protocols and escalating concerns.

Tactical Execution & Escalation

  • Use data and KPIs to identify team performance trends and operational risks; escalate as needed.
  • Resolve day-to-day operational challenges within scope.
  • Support the Director in addressing technical or clinical issues impacting care delivery.

Peer Coordination & Support

  • Collaborate with clinical teams, including Physicians, Nurse Practitioners, and Social Workers, to support patient care plans.
  • Communicate team priorities and updates during operational meetings.

Change Implementation & Feedback

  • Support implementation of new care coordination tools and processes.
  • Gather team feedback to inform process improvements and share with leadership.

Qualifications:

  • Bachelor’s degree in Healthcare, Health Administration, Business, or a related field required, or equivalent professional experience.
  • 3–5 years of experience in healthcare operations, preferably in geriatrics, dementia, or chronic care.
  • At least 1 year in a lead, supervisory, or senior specialist role with responsibility for guiding others.
  • Reliable high-speed internet connection required to support telehealth operations and virtual collaboration while prioritizing PHI.
  • Strong proficiency with EHR systems (Athena preferred), G-Suite, and telehealth tools.
  • Knowledge of HIPAA and clinical compliance standards.
  • Excellent communication skills and ability to work collaboratively in a remote, fast-paced environment.
  • Passion for dementia care and supporting caregivers through thoughtful coordination.

Pay Range Details

The pay range(s) below are provided in compliance with state specific laws. Pay ranges may be different in other locations. Exact compensation may vary based on skills, experience, and location.

Role: $60,000 - $78,000 depending on experience

Offer of employment is contingent upon successful completion of a background check, drug screen, and all required licensure, credentialing, and related documentation.
